Your first consultation will require a longer appointment to allow time for us to discuss your medical history and understand your individual healthcare needs.

Please arrive 10 minutes early to complete your new patient form. Alternatively, you can complete the form prior to your appointment.
Telephone Appointments

Cockatoo Family Medical Practice offers telehealth appointments to existing patients, subject to Medicare eligibility requirements.

To receive a Medicare rebate for a telehealth consultation, you must have had a face-to-face consultation with one of our doctors within the past 12 months. If you have not attended the clinic in the past 12 months, the full consultation fee will apply. Please note that telehealth may not be appropriate in these circumstances, and this will be determined at the doctor’s discretion.

Care Management Plans

If you are eligible for a Chronic Disease Management Plan, you will need to make an appointment with our Practice Nurse to complete the necessary paperwork and health assessments. This plan may provide access to government-subsidised allied health services, including podiatry, dietetics, physiotherapy and more.

Bulk Billing
Bulk billing is available at Cockatoo Family Medical Clinic for:

For all other patients, Cockatoo Family Medical Clinic operates as a private billing practice. Please see the fee schedule below for applicable out-of-pocket costs.

Please note that Medicare rebates are processed automatically when payment is made using a debit card. If you pay by credit card or cash, you will need to process your Medicare rebate online after your appointment.

A 1.3% surcharge applies to all debit and credit card payments.

If you are experiencing financial difficulties, please speak with your doctor for temporary financial assistance during times of hardship.

Please note that private licences, including truck licences, may require an initial general consultation before the required assessment can be completed.
